Functional Assessment Instrument Attributes
Description:
OASIS (Outcome and Assessment Information Set). Created by the Center for Health Services Research at the University of Colorado under contract to the Centers for Medicare and Medicaid Services (CMS, formerly Health Care Financing Administration HCFA) to monitor quality and to determine payment for Medicare recipients using home care services. Functional status is a subset of the assessment. The OASIS is not meant as a comprehensive assessment instrument but as a tool to measure patient outcomes. OASIS is a subset of the full home care assessment that Home Health Agencies must complete. Under Medicare rules Home Health Agencies must do a comprehensive assessment which must include the OASIS.

Prevalence:
Mandatory for patients receiving home care who are Medicare or Medicaid beneficiaries. Some home care agencies may use for non-Medicare patients. Done at start of care, resumption of care following inpatient facility stay, a periodic (60 day) follow-up, and on discharge or transfer from home care.
Population:
Adult patients (18 and older)receiving home care services. Patients receiving services for antepartum or postpartum care are excluded.
