An 80 year-old woman is being discharged from the nursing home after residing there for a period of three months. She had been admitted as it was determined that she could not manage at home following a hip fracture. She received an extensive rehabilitation program and at this time she is able to ambulate for at least 100 feet without any assistance. Also at the time she was discharged from the hospital she exhibited a significant degree of confusion. Presently her mental status has returned to normal. Her home situation is judged to be safe both respect to caregiver support (her daughter) and the physical environment.
