Mr. J resides in a long-term care facility. He suffers from ESRD as well as diabetes, hypertension, and dysphagia. Mr. J receives dialysis at a local dialysis center three times a week. Under contract with the dialysis center as required by federal regulation, the long-term care facility and dialysis center must coordinate Mr. J's plan of care including the sharing of medication usage, lab values, dietary recommendations. Mr. J's dietitians at both facilities fax information to each other three times a week to manage his renal disease.
