A Home Care Agency and a Patient

Use Case Scenario

The following scenario is based on a U.S. setting but can be generalized. Just substitute for OASIS the relevant assessment done on discharge from home care.

A person receives home care services for a specified time. On discharge from home care, the home care agency completes an OASIS discharge assessment. This assessment contains the following functional status information:

sensory status

integumentary status

respiratory status

elimination status

neuro/emotional status

ADL/IADL

patient management of medications and equipment

The home care agency encodes the functional status information (along with other information on the patient) into a CCD. This CCD is stored onto some form of electronic storage and/or printed. The electronic and/or printed medium is given to the patient to use as a personal health record.

Use Case: A person receives home care for a certain period. During that time it is found that this person can no longer independently use utensils to feed himself. This information is recorded on the OASIS and the personal health record of the patient. The patient at some later time, after they have been discharged from home care, is admitted to a hospital. Using his personal health record the hospital staff know that when this person is eating that an aid needs to be present to assist.
